Please submit the form below. The "Team Conflict" field below tells us know that you share players with another team and we should try to work around scheduling these teams at the same time. Only 1 request can be honored due to the number we receive. If you have special circumstances that require scheduling consideration, please email us separately so we can consider it. Not all requests can be worked into the schedules. You will be directed to a thank you page letting you know that your form was accepted.
League Fees: 6's = $300 (9 matches) 4's = $200 (9 matches)
*Ref fees are $10 per match
League fees are due by your 2nd match. Make checks payable to Sports Express. Schedules will be posted on the website. Please go to the standings page to find your game times.
